New patients are welcome and we will make every attempt to find you an appointment soon.

When you arrive, bring your insurance card so that we may make a copy.

Forms: (We hate forms too): However, like all offices, some are required, including by the U.S. Government (HIPAA form - verifying how you want us to handle your private medical information). All new patients will be asked to complete the medical history form (this may be downloaded from the "Form" section of this website.

We ask that all new patients arrive a minimum of 10 minutes prior to their scheduled appointment in order to complete the registration process.

If cancellation or rescheduling is necessary please give us 24 hours notice, as there are often other patients who would desire to take an available appointment time.
